This arbovirus, belonging to the Flaviviridae family, is characterised by sudden onset of fever, severe joint and muscle pain, rash, and other flu-like symptoms.

Dengue fever is a viral infection transmitted to humans by the bite of infected Aedes mosquitoes, particularly Aedes aegypti. It is most prevalent in tropical and subtropical regions of the world, especially in parts of Southeast Asia, South America, the Caribbean, Africa, and the Pacific Islands. The disease is caused by one of four closely related dengue viruses, and a person can be infected with each of them over their lifetime. Dengue is sometimes referred to as “breakbone fever” due to the severe muscle and joint pain it can cause. While many cases are mild, dengue can lead to serious complications, particularly dengue haemorrhagic fever or dengue shock syndrome, which may be life-threatening without prompt medical care.

Dengue is spread through the bite of an infected mosquito, not from person to person. The Aedes mosquito is most active during the early morning and late afternoon and tends to breed in stagnant water around homes, such as in plant pots, tyres, and containers. Prevention is based on avoiding mosquito bites. Travellers should wear long sleeves and trousers, use insect repellent containing DEET, sleep under mosquito nets if necessary, and stay in accommodation with air conditioning or screened windows. Eliminating standing water and using plug-in vaporisers or coils can also reduce mosquito presence. A vaccine for dengue, called Qdenga, is available in some countries, including the UK, and may be recommended for travellers with a history of prior dengue infection or those spending extended periods in high-risk areas.

Symptoms of dengue fever typically develop 4 to 10 days after being bitten by an infected mosquito and often begin suddenly. Early signs include high fever, severe headache (especially behind the eyes), muscle and joint pain, nausea, vomiting, and a skin rash. The illness usually lasts for about a week. Some individuals may recover quickly, while others experience a second phase where symptoms worsen. Warning signs of severe dengue include bleeding gums, blood in vomit or stools, abdominal pain, difficulty breathing, and persistent vomiting. These symptoms require urgent medical attention, as they may indicate internal bleeding, shock, or other complications.

The dengue vaccine, Qdenga, provides protection against all four dengue virus types and is approved for use in several countries, including the UK. It is typically given as two doses, three months apart. Studies suggest that it offers protection for at least four years, and possibly longer, though ongoing research is evaluating its long-term effectiveness. The vaccine is primarily recommended for people aged 4 to 60 who have had a previous dengue infection and are travelling to or living in high-risk areas. Vaccination without prior exposure may carry a higher risk of severe disease, so testing for previous infection is often recommended before vaccination.

Dengue fever is endemic in more than 100 countries, and the vaccine may be advised for travellers visiting parts of Southeast Asia (such as Thailand, Indonesia, and the Philippines), the Indian subcontinent, Central and South America (including Brazil and Colombia), the Caribbean, Africa, and some Pacific islands. Long-term travellers, expatriates, and aid workers are most likely to benefit from the vaccine, especially if they have had a previous dengue infection. Holidaymakers on short stays are usually advised to rely on mosquito bite prevention, unless they have a known history of dengue.

The Qdenga vaccine is generally well tolerated, with most side effects being mild and resolving quickly. Common side effects include injection site pain or swelling, headache, fever, fatigue, and muscle aches. Some people may also experience a mild rash or gastrointestinal symptoms. Serious side effects are rare but may include allergic reactions. Because the vaccine is live and attenuated, it is not suitable for pregnant women, immunocompromised individuals, or those with certain medical conditions. Prior consultation with a travel health specialist or GP is essential to determine eligibility and ensure the vaccine is appropriate based on individual health status and travel plans.
